The Solution
PCE is an analytics and communication system built to identify cohorts in a larger population by applying advance dynamic criteria. Dashboards showing cohort statistics were designed:
- Patient 360° view: Detailed patient history to provide personalized healthcare.
- Chronic disease patient cohort view: Full report of patient groups with particular diseases including their visits, diagnoses, tests and results, and gaps in care.
- fDiabetes population monitoring: Monitoring of the financial impact of pre-diabetes patients, and the patients enrolled in a diabetes management program.
- Patient engagement: Appointment reminders and rescheduling of missed appointments.
